Iowa Medicare Plans Ending in 2027: Who Is Affected and What to Do

The Iowa Insurance Division reports that more than 87,000 Iowans received notice that their Medicare Advantage, Cost, or Part D plan will not continue in their county for 2027. The biggest single change is Wellmark ending all of its Medicare Advantage coverage in Iowa, not just trimming counties. There is also a consequence that is easy to miss until you need care: with the Wellmark and Medica PPOs both gone, no Iowa Medicare Advantage plan is expected to carry Mayo Clinic in-network for 2027. If your plan is ending you get a protected window to switch and a rare chance to buy a Medigap policy without medical underwriting. Here is exactly what to do, and by when.

What best describes your situation?

Your plan ending opens a door that is usually closed

When your plan is non-renewing, you get a Special Enrollment Period from December 8 through the last day of February 2027, on top of the regular Annual Enrollment Period (October 15 to December 7). More valuable still: if you move to Original Medicare, you have guaranteed-issue Medigap rights, generally from 60 days before your coverage ends to no later than 63 days after. During that window an insurer cannot refuse you or charge you more for your health history. Iowa’s SHIIP-SMP director called the ability to switch to Original Medicare with guaranteed issue a rare opportunity, and that is not marketing language: outside a guaranteed-issue window, Iowa insurers can medically underwrite Medigap applications, so a window you let lapse may not come back at any price.

Which Iowa plans are ending

The Iowa Insurance Division put the total at more than 87,000 notices across Medicare Advantage, Cost, and Part D plans. The headline changes:

Wellmark: all Medicare Advantage coverage ends

Wellmark is ending its entire Medicare Advantage line in Iowa (and South Dakota) after December 31, 2026. This is a full exit rather than a county-level pullback, which is why the affected number is so large. Wellmark Medicare Advantage members must actively choose something else.

Medica: Medicare Advantage PPO ends in western Iowa

Medica is ending Medicare Advantage PPO coverage in Harrison, Mills, and Pottawattamie counties.

UnitedHealthcare and Humana: individual plans end

A UnitedHealthcare HMO-POS plan and a Humana PPO plan are also ending, and other plans are shrinking their service areas county by county.

Cost and Part D plans are in the mix too

The 87,000 figure is not Medicare Advantage alone. Medicare Cost plans and standalone Part D drug plans are also affected, and losing a Cost plan is a different kind of loss (see below).

SHIIP-SMP publishes the complete list of affected plans and counties at shiip.iowa.gov. Check your own county rather than assuming, because several plans are changing service areas without disappearing statewide.

If you go to Mayo Clinic, read this before you pick a plan

No Iowa Medicare Advantage plan is expected to carry Mayo Clinic in-network in 2027.

The Wellmark and Medica Medicare Advantage PPOs were the plans that gave Iowans in-network access to Mayo Clinic in Rochester, Minnesota. With both ending, the Iowa Insurance Division reported that Iowans will no longer be able to buy a Medicare Advantage plan with in-network Mayo coverage for 2027. For northern Iowa especially, where Mayo is the referral destination for complex care, this can matter more than premiums.

If Mayo access is the reason you hold the plan you hold, the honest answer is that Original Medicare is the route to price out. Mayo accepts Original Medicare, and a Medigap policy can cover most of the cost sharing that comes with it. That path is open to you right now without medical underwriting precisely because your plan is ending, and it narrows once the guaranteed-issue window closes.

Whatever you choose, confirm network status with the plan and with Mayo directly before you enroll. Plan directories are frequently out of date, and in late 2026 some Medicare Advantage directories were still listing hospitals as in-network weeks before those hospitals were scheduled to leave. See our guide on how to check whether a provider is really in-network.

Losing a Medicare Cost plan is not the same as losing Medicare Advantage

Iowa is one of the few places where this distinction still comes up. A Medicare Cost plan is an older type of Medicare health plan authorized under Section 1876 of the Social Security Act, and its defining feature is flexibility: if you receive covered Part A or Part B services outside the plan’s network, those services are still covered by Original Medicare, with Original Medicare’s deductibles and coinsurance. A Medicare Advantage plan generally does not work that way, and out-of-network care can cost far more or not be covered at all.

Cost plans are nearly extinct. As of 2025 only about 11 Cost plan contracts remained nationwide, most of them serving parts of Minnesota, Wisconsin, Iowa, the Dakotas, and a single Illinois county, after the 2015 MACRA law accelerated their phase-out.

The practical point: if your Cost plan is ending, do not assume a Medicare Advantage plan is an equivalent swap. You would be trading away out-of-network flexibility you may have been relying on without realizing it. Compare it honestly against Original Medicare with a Medigap policy, which preserves that freedom of provider choice.

The three dates that matter

1

December 7, 2026

The Annual Enrollment Period closes. Choosing by this date is the cleanest path, with coverage starting January 1.

2

December 31, 2026

The Iowa Insurance Division flags this as the practical deadline: the last day someone whose plan was cancelled can choose coverage that begins January 1 without a gap.

3

February 28, 2027

The Special Enrollment Period for a non-renewed plan ends. Enrolling in January or February means coverage starts the first of the following month, so you can be briefly uncovered. Your Medigap guaranteed-issue window runs on its own clock, generally ending no later than 63 days after your coverage ends.

Do not wait for the deadline. When a large plan exits a state at once, the remaining plans take on a surge of new members, and popular plans can close enrollment or get slow to process. Acting in the Annual Enrollment Period rather than the February tail leaves you room to fix a problem.

Have us find your new Iowa plan

With Wellmark out of Medicare Advantage entirely, the real question is which of the plans still available in your county keeps your doctors, covers your prescriptions, and gets you to the hospital you actually use, and whether Original Medicare plus a Medigap policy is the better fit while guaranteed issue is open to you. For a flat $99, CareRoute Find My Medicare Plan researches that for you and verifies provider participation with the offices directly rather than trusting a directory. We are independent, do not sell insurance, and take no commissions.

Frequently asked questions

How do I know if my Iowa Medicare plan is ending in 2027?

If your plan is not being offered in your county next year, your insurer was required to mail you a non-renewal notice, generally by early October. Read it, because it is easy to mistake for junk mail. Also read your Annual Notice of Change, mailed by September 30, which spells out next year’s costs and coverage. To confirm directly, compare plans for your ZIP code on the Medicare Plan Finder at Medicare.gov, check the SHIIP-SMP list of affected plans and counties at shiip.iowa.gov, or call SHIIP at 1-800-351-4664. You can also upload your notice to our free Medicare Letter Check and we will explain exactly what it means.

Which Iowa Medicare plans are ending for 2027?

The Iowa Insurance Division says more than 87,000 Iowans got notices that a Medicare Advantage, Cost, or Part D plan will not continue in their county for 2027. The largest single change is Wellmark, which is ending all of its Medicare Advantage coverage in Iowa (and South Dakota) after December 31, 2026, rather than pulling back from a few counties. Medica is ending Medicare Advantage PPO coverage in Harrison, Mills, and Pottawattamie counties, and a UnitedHealthcare HMO-POS plan and a Humana PPO plan are also ending. Other plans are shrinking their service areas county by county. The complete list of affected Medicare Advantage, Cost, and Part D plans by county is published by SHIIP-SMP at shiip.iowa.gov.

Is it true that no Iowa Medicare Advantage plan will cover Mayo Clinic in-network in 2027?

That is what the Iowa Insurance Division reported when open enrollment opened. Wellmark and Medica were the Medicare Advantage PPOs that carried in-network Mayo Clinic coverage for Iowans, and with both of those PPOs ending, no Medicare Advantage plan sold in Iowa is expected to include Mayo Clinic as in-network for 2027. This matters most in northern Iowa, where many people travel to Rochester, Minnesota for specialty care. If Mayo access is the reason you chose your plan, Original Medicare is the route worth pricing, because Mayo accepts Original Medicare and a Medigap policy can cover most of the cost sharing. Networks can change, so confirm directly with both the plan and Mayo before you enroll rather than relying on a plan directory.

My Iowa Medicare Advantage plan is ending. Can I buy a Medigap policy without medical underwriting?

In most cases yes, and this is the part people miss. When your Medicare Advantage plan is non-renewing and you return to Original Medicare, federal rules give you a guaranteed-issue right to buy certain Medicare Supplement (Medigap) policies without medical underwriting, generally from 60 days before your coverage ends to no later than 63 days after. An insurer cannot turn you down or charge you more because of your health history during that window. As Iowa’s SHIIP-SMP director put it, the ability to switch to Original Medicare with guaranteed issue is a rare opportunity. Outside a guaranteed-issue window, Iowa insurers can medically underwrite Medigap applications, so letting this window lapse can mean you cannot get the policy later at any price.

What is a Medicare Cost plan, and is losing one different from losing a Medicare Advantage plan?

Yes, it is different, and Iowa is one of the few places this still comes up. A Medicare Cost plan is an older type of Medicare health plan authorized under Section 1876 of the Social Security Act. Its defining feature is flexibility: unlike Medicare Advantage, if you get covered Part A or Part B services outside the plan’s network, those services are still covered under Original Medicare, with Original Medicare’s deductibles and coinsurance. You can also generally join or leave outside the usual windows. Cost plans are nearly extinct, with only about 11 contracts left nationwide as of 2025, mostly in Minnesota, Wisconsin, Iowa, the Dakotas, and one Illinois county, after the 2015 MACRA law accelerated their phase-out. If your Cost plan is ending, you are losing that out-of-network flexibility, so compare carefully before assuming a Medicare Advantage plan is an equivalent replacement.

If my Iowa plan is ending, when exactly can I switch?

You have more than one window, and the dates matter. The Annual Enrollment Period runs October 15 to December 7, 2026, with coverage starting January 1. Because your plan is non-renewing, you also get a Special Enrollment Period that runs December 8 through the last day of February 2027. The Iowa Insurance Division flags December 31, 2026 as the practical deadline: it is the last day someone whose plan was cancelled can choose new coverage that starts January 1 without a gap. If you enroll in January or February instead, your coverage starts the first of the following month, which can leave you briefly uncovered. Separately, mind the Medigap guaranteed-issue window described above, which runs on its own clock.
