  • Proof of household size (tax return showing dependents, birth certificates, or lease agreement)
  • Government-issued ID (driver's license, state ID, or passport). Citizenship is not required.
Tip: Jackson Health requires Miami-Dade County residency for its financial assistance program. Be prepared to show proof of address (such as a utility bill or lease) when applying there.
