What is FPL? The Federal Poverty Level is a government-set income threshold used by hospitals to determine who qualifies for free or discounted care. For example, 400% FPL for a family of four is about $132,000 per year. The higher a hospital's FPL threshold, the more people qualify.

What You Need to Apply

Most Dallas-Fort Worth hospitals require similar documentation. Gather these before you apply:

  • Proof of income for all household members (recent pay stubs, tax returns, Social Security or disability award letters, or a signed statement of no income)
  • Proof of household size (tax return showing dependents, birth certificates, or lease agreement)
  • Government-issued ID (driver's license, state ID, or passport). Citizenship is not required.
  • Bank statements (last 3 months). Required by most DFW hospitals, including Baylor Scott & White, Medical City, and Texas Health Resources.
Tip: Parkland Health requires proof of Dallas County residency (utility bill, lease, or TX ID with Dallas County address). Check each hospital's specific requirements in their full guide before applying.

Texas Law Protections

Texas has fewer state-level hospital billing protections than some other states. However, federal 501(r) rules still require nonprofit hospitals to offer financial assistance. Key points for Dallas-Fort Worth patients:

  • No state charity care mandate: Texas does not have a state law requiring specific charity care thresholds. Nonprofit hospitals must follow federal 501(r) rules, which require a written financial assistance policy but do not set minimum FPL levels.
  • Homestead protection: Texas has strong homestead protections. Hospitals cannot force the sale of your primary residence to collect a debt, though urban homesteads are limited to 10 acres.
  • Hospital liens: Texas hospitals can place liens on personal injury claims and, in some cases, on property. Review any lien notices carefully.
  • 4-year statute of limitations: Hospitals and debt collectors have 4 years from the date of last activity to file a lawsuit to collect a medical debt.
  • Surprise billing protections: Texas has state-level protections against surprise (out-of-network) billing for emergency care and some in-network facility situations.
