Novant Health provides 100% free care for patients at or below 300% FPL, and as of January 2025, insured patients also qualify. The system has also launched a retroactive debt relief program that zeros out unpaid debt from 2014 forward for NC Medicaid patients.

Who Qualifies for Free Care

Novant Health offers one of the most generous financial assistance policies among major health systems. Patients with household income at or below 300% of the Federal Poverty Level receive 100% free care for medically necessary services.

Key Change (January 2025)

Novant Health expanded eligibility to include insured patients, not just uninsured. If you have health insurance but still cannot afford your out-of-pocket costs (copays, deductibles, coinsurance), you may now qualify for free care at 300% FPL. This is a significant expansion that most patients are unaware of.

This threshold is notably higher than many peer systems. For reference, a family of four earning up to approximately $96,300 per year would fall at or below 300% FPL. Single individuals earning up to approximately $47,790 would qualify.

Retroactive Debt Relief Program

Novant Health has committed to one of the most aggressive debt relief programs in the country. Here is what it includes:

What is Being Zeroed Out

  • All unpaid medical debt from January 1, 2014 forward for North Carolina residents who were on Medicaid at the time of service
  • Applies retroactively, meaning debt that has already been sent to collections may be recalled and eliminated
  • No application required for patients automatically identified through Medicaid records

Starting July 2025: Automatic Screening

Beginning July 2025, Novant Health automatically evaluates all North Carolina Medicaid patients for past unpaid debt within 60 days of hospital discharge. You do not need to apply. If you have qualifying unpaid balances, they will be identified and zeroed out proactively.

The 5% Income Rule

Even if your income exceeds 300% FPL, you may still qualify for Novant Health's medical debt relief program under the 5% income threshold:

  1. Income at or below 350% FPL (approximately $55,755/year for a single person, or $112,350 for a family of four)
  2. Medical debt at Novant Health exceeds 5% of your annual household income

Example: A family of four earning $90,000/year (below 350% FPL) with $5,000 in Novant Health medical debt would qualify because $5,000 exceeds 5% of $90,000 ($4,500).

This provision is especially valuable for patients who have insurance but have accumulated significant out-of-pocket costs through deductibles and coinsurance over multiple visits.

Financial assistance also covers Novant Health-affiliated physician clinics, urgent care centers, and outpatient facilities that bill under the Novant Health system. Independent physicians with admitting privileges who bill separately are not covered.

How to Apply

Confirm Your Eligibility

Check if your household income falls at or below 300% FPL for free care, or 350% FPL for the debt relief program. Use the FPL table above. Remember that insured patients now qualify.

Contact Patient Financial Services

Call 1-888-277-1015 or ask for a financial counselor during your hospital visit. You can also visit novanthealth.org to download the application.

Gather Documentation

Prepare proof of income (recent pay stubs, tax return, SSI/SSDI award letter, or unemployment documentation), photo ID, and your insurance card if applicable.

Submit Your Application

Submit by mail, fax, in person at any Novant Health facility, or through your MyNovant online portal. Apply within 240 days of your first billing statement for best results.

Follow Up

Call 1-888-277-1015 to confirm receipt. Novant Health must provide a determination within a reasonable timeframe. If you do not hear back within 30 days, follow up.

If You Are Denied

If Novant Health denies your financial assistance application, you have several options:

  • Request written reason for denial. Novant Health must provide a written explanation of why your application was denied.
  • Appeal with additional documentation. If your income has changed, you have new hardship circumstances, or documentation was missing, resubmit with updated proof.
  • Check the 5% income rule. Even if denied under the 300% FPL threshold, you may qualify at 350% FPL if your debt exceeds 5% of income.
  • Request Medicaid screening (NC). Under North Carolina law, hospitals must screen patients for Medicaid eligibility before pursuing collections.
  • File a complaint. In NC, contact the NC Attorney General's office. In SC, contact the SC Department of Health. In VA, contact the Virginia Health Commissioner.

Novant Health cannot send your account to collections while a financial assistance application or appeal is pending. If they do, this is a violation of their own policy and potentially of state law.
