Baystate Health Financial Assistance: Free Care Under 204% FPL + Massachusetts HSN Safety Net (2026 Guide)
Baystate Health is Western Massachusetts's largest healthcare system with 5 facilities including the region's only Level I trauma center. Patients under 204% FPL qualify for free care, and the Massachusetts Health Safety Net (HSN) fund provides additional coverage for patients up to 300% FPL. One application screens for MassHealth, Commonwealth Care, and HSN simultaneously. Massachusetts offers some of the strongest medical debt protections in the country, including a $1M homestead exemption and a 6-year statute of limitations.
Quick Eligibility Snapshot
Free Care: Under 204% FPL
Baystate's own financial assistance provides 100% free care for households under 204% FPL. Family of 4: up to ~$64,200/year (2025 guidelines).
HSN: 0-150% FPL = Zero Out-of-Pocket
Massachusetts Health Safety Net reimburses the hospital directly. Patients at 0-150% FPL should have ZERO patient liability. The state pays the hospital.
Discounted Care: Under 322% FPL
Baystate provides discounted care for households between 204% and 322% FPL. HSN also covers 150.1-300% FPL with a deductible.
One Form, Multiple Programs
The Medical Benefit Request form evaluates you for MassHealth, Commonwealth Care, AND Health Safety Net all at once. File once, get screened for everything.
Who Qualifies for Financial Assistance?
Dual System: Baystate FA + Massachusetts Health Safety Net
- Baystate Free Care: Household income under 204% FPL = 100% write-off of eligible hospital charges.
- Baystate Discounted Care: Household income 204-322% FPL = discounted rate on eligible hospital charges.
- HSN Full Coverage: 0-150% FPL = the state reimburses the hospital directly with no deductible. You owe nothing.
- HSN Partial Coverage: 150.1-300% FPL = state coverage with a deductible. Still significantly reduces your liability.
- Key strategy: If you are between 204-300% FPL, pursue HSN determination FIRST. HSN may provide full or near-full coverage even though Baystate's own program only offers a discount at that level.
Important: Physician Bills Are NOT Covered by HSN
The Health Safety Net covers hospital charges only. Baystate Medical Practices bills physician services separately. For physician billing questions or financial assistance for doctor charges, call 413-794-5700 or 800-436-6865. You may need to negotiate these bills independently.
How to Apply (Step by Step)
Get the Medical Benefit Request Form
Contact Baystate Financial Counselors at 413-794-2452 or visit any Baystate Health facility. This single form screens you for MassHealth, Commonwealth Care, and the Health Safety Net simultaneously.
Gather Documents
Collect proof of income (recent pay stubs or tax returns), proof of Massachusetts residency, photo identification, and documentation of household size. If unemployed, bring documentation of any benefits you receive.
Submit to Financial Counselors
Submit in person at any Baystate facility or by calling 413-794-2452. Financial counselors can help you complete the form and determine which programs you may qualify for.
Wait for Determination
Your application will be evaluated for multiple programs. If approved for HSN at 0-150% FPL, the hospital is reimbursed by the state and you owe nothing. At higher FPL levels, you will receive a determination letter explaining your coverage or discount.
Address Physician Bills Separately
If you also received physician bills from Baystate Medical Practices, contact them at 413-794-5700 or 800-436-6865 to request financial assistance for those charges. HSN does not cover physician fees.
Pro Tips
- Dollar For will help for free: The nonprofit dollarfor.org helps patients with financial assistance applications at no cost.
- Between 204-300% FPL? Pursue HSN first. HSN may cover your entire bill even though Baystate's own program only offers a discount at that income level.
- MassHealth can be retroactive: MassHealth coverage can be applied retroactively up to 3 months, potentially covering bills you already received.
- Apply before or during your visit: Financial counselors are available at Baystate facilities and can process applications even before you receive a bill.
Massachusetts State Protections for Medical Debt
Massachusetts has some of the strongest consumer protections against medical debt in the country. The state is 97% insured, has a robust safety net fund, and provides substantial asset protections.
$1M Homestead Protection
Massachusetts provides an automatic $125,000 homestead exemption on your primary residence. If you file a declaration, protection increases to $1 million. Medical debt judgments cannot force the sale of your home up to these limits.
6-Year Statute of Limitations
Creditors have 6 years to file a lawsuit on medical debt in Massachusetts. After 6 years without a payment or lawsuit filing, the debt is time-barred and cannot be collected through the courts.
HSN Safety Net Fund
The state-funded Health Safety Net reimburses hospitals for care provided to qualifying patients. This reduces the hospital's incentive to pursue patients aggressively, since the state covers a significant portion of uncompensated care.
Pending Legislation: H.4809 (Expected 2025-2026)
New Massachusetts legislation would prohibit the sale of medical debt to third-party collectors, protect $5,000 in cash and $15,000 in vehicle value from seizure for medical debt. This would make Massachusetts one of the most protective states in the country for patients with medical bills.

Covered Facilities
Baystate Health Facilities (5 Locations)
Baystate Health is the largest healthcare system in Western Massachusetts, serving over 800,000 people across a four-county region. The financial assistance policy applies to all Baystate Health facilities.
Western MA Context
Springfield has significant poverty rates, making financial assistance programs especially important in this region. Many patients in the Baystate service area will qualify for free care or HSN coverage. If you live in Western Massachusetts and received care at any Baystate facility, there is a strong chance you qualify for help.
Collections Policy
Low Risk: No Evidence of Aggressive Collections
There is no specific evidence of Baystate Health filing lawsuits against patients for unpaid medical bills. Massachusetts's strong consumer protections, the HSN safety net fund, and pending legislation (H.4809) all work together to create a low-risk environment for patients with medical debt.
Why Massachusetts Is Different
- 97% insured: Massachusetts has the highest insurance rate in the country, reducing uncompensated care
- HSN reimburses hospitals: The state fund pays hospitals for charity care, reducing their incentive to pursue patients
- $1M homestead: Even if sued, your home is protected up to $1 million (with filed declaration)
- 6-year SOL: After 6 years, medical debt is time-barred from lawsuits
- H.4809 (pending): Would ban the sale of medical debt and protect additional assets
Massachusetts Protections Against Collections
- Automatic $125K homestead: No filing required for baseline protection
- $1M declared homestead: File a declaration at your registry of deeds for maximum protection
- 6-year SOL: Medical debt claims expire after 6 years
- HSN coverage: If you qualify, the state pays and you owe nothing

What If You Are Denied?
1. Check if HSN Still Applies
Even if denied Baystate's own financial assistance, you may still qualify for Health Safety Net coverage. HSN has different eligibility criteria. Ask specifically about your HSN determination and whether it was evaluated separately.
2. Appeal or Reapply
If denied for missing documents, ask if you can supplement your application. If your financial situation has changed (job loss, medical emergency), provide updated documentation and request reconsideration.
3. Contact MassHealth Directly
Call MassHealth/HSN at (800) 841-2900. MassHealth coverage can be retroactive up to 3 months, potentially covering bills you have already received. A separate determination from the state may yield different results.

Frequently Asked Questions
What is the difference between Baystate FA and the Health Safety Net?
Baystate's own financial assistance (charity care) is the hospital's internal program. The Health Safety Net (HSN) is a Massachusetts state program that reimburses hospitals directly from a state fund. Both reduce or eliminate your bill, but HSN is funded by the state rather than the hospital absorbing the cost. One application screens for both.
I am between 204% and 300% FPL. What should I do?
This is a crucial range. Baystate's own program only offers discounted care above 204% FPL, but the Massachusetts HSN may provide full or near-full coverage up to 300% FPL. Make sure your Medical Benefit Request form is evaluated for HSN specifically. If only offered a Baystate discount, ask about your HSN determination.
Does HSN cover my doctor's bill from Baystate?
No. The Health Safety Net covers hospital facility charges only. Physician charges from Baystate Medical Practices are billed separately and are NOT covered by HSN. You will need to contact Baystate Medical Practices at 413-794-5700 or 800-436-6865 for help with physician charges.
Can Baystate Health put a lien on my home?
Massachusetts provides automatic homestead protection of $125,000 (up to $1 million with a filed declaration). Even if a medical debt judgment is obtained, your primary residence is protected up to these limits. There is no evidence of Baystate Health pursuing aggressive collection litigation.
I do not have insurance. Does Baystate still have to treat me?
Yes. Under federal EMTALA law, Baystate Medical Center (and all emergency departments) must provide emergency stabilization regardless of ability to pay. Beyond emergencies, Massachusetts's near-universal coverage means most residents can obtain MassHealth or marketplace insurance. Financial counselors at 413-794-2452 can help you explore options.
