What You Need to Apply
Most Chicago hospitals require similar documentation. Gather these before you apply:
- Proof of income for all household members (recent pay stubs, tax returns, Social Security or disability award letters, or a signed statement of no income)
- Proof of household size (tax return showing dependents, birth certificates, or lease agreement)
- Government-issued ID (driver's license, state ID, or passport). Citizenship is not required.
Illinois Law Protections
Illinois has strong hospital billing protections for patients. Key laws that protect Chicago patients:
- Hospital Uninsured Patient Discount Act (210 ILCS 89): Hospitals must provide 100% free care to uninsured patients at or below 200% FPL for medically necessary services exceeding $300. Discounts are required for uninsured patients up to 600% FPL.
- Medical debt credit reporting ban (Public Act 103-0648): As of January 1, 2025, credit reporting agencies cannot include medical debt on consumer credit reports. Hospitals cannot damage your credit score with unpaid medical bills.
- 180-day collection protection: Hospitals cannot report medical debt to credit agencies for at least 180 days, giving you time to apply for financial assistance or set up a payment plan.
- Annual payment cap: For uninsured patients qualifying under the Act, total payments in any 12-month period cannot exceed 20% to 25% of family income, depending on the hospital.
